Thank you for choosing the Patient Portal to stay connected with your care team. To register for the patient portal, the Patient Portal Request Form will need to be filled out and submitted.

Who can sign the form?

  • If the patient is unable to sign this form, the patient’s guardian or involved family member can sign and submit this form.
  • If the patient resides in a group home, the CEO or Medical Director can sign and submit this form or please see instructions below.

Access to Patient Portal for Group Homes:

  • A letter signed by the CEO or Medical Director to authorize a member of the Clinical Staff to have access to the Patient Portal is also acceptable.
  • Please notify us by sending an email to pportal@wihd.org if a member of your staff with access to the Patient Portal leaves the group home.

What happens after I submit the form?

Once the “WIHD Patient Portal Request Form” is submitted, it will go through a review and approval process. Once approved you will receive two emails:

  • One with the patient portal user name
  • Second e-mail with the password.
