How will my records be released?

There are several methods WIHD can use to release records.

  1. Patient Portal – for most clinical health services (not including Early Intervention) WIHD’s Patient Portal is available for every patient.  The patient portal has the following benefits:  secure, some health information is available, visible past and future visit schedule, and records can be requested. If you would like more info about the WIHD Patient Portal or to request access please click here.
  2. Fax, mail, secure email –Records can be sent via these methods as long as this information is in the Consent for Disclosure of Protected Health Information (PHI) Form (also available in Spanish) signed by the patient or legal guardian/representative.
  3. In Person –Records can be picked up in person by the patient or an authorized individual.  A picture ID will be required before records are released.

How long does it take to get my records?

All requests are worked in the order they are received.  While effort is made to complete requests as soon as possible please allow up to 7-10 business days to process.
