LIFEPlan CCO NY: Care Management Explained

This session explains how Care Management is the key to coordinating OPWDD supports and services for people with intellectual and developmental disabilities. Families and people with IDD benefit through connection to a care manager who is assigned to help them find, coordinate, and access services, including healthcare, social supports, disease and chronic condition care, and access to preventative care.
