Parent and Student Plain Language IEP Form

Students Studying In Classroom With Digital Devices

The IEP document is not exactly friendly. The language of the official document is, well, official. Even the font is a bit intimidating! The WIHDCSN has created this form to help parents and students understand the document and what goes in it. This resource will help students and families to understand the process of IEP development and even share their own ideas in a format that makes sense.

Click on the resources below. Please note, the first two will download as a Word documents, not redirect to a new page. Please check where you would normally find downloaded items on your computer (at the bottom of the web browser, in the “Download” folder of your computer, etc.)
