Westchester’s Medical Home for People with Intellectual and Developmental Disabilities
MEDICAL HOME RESPONSIBILITIES
The Medical Home coordinates patient services across the healthcare spectrum. We coordinate care among our providers at WIHD and with community-based specialists, behavioral health resources, rehabilitation facilities, hospitals, and emergency rooms.
The Medical Home assists with collection of personal health records from previous and current medical providers for both adult and pediatric transitions. Patients and providers can contact our office at 914-493-8148 to transfer existing records to our practice. We also provide education and assistance to individuals regarding access to health insurance coverage.
At your visits, we take your personal preferences and health goals seriously. We use evidence based practices in providing acute, chronic, and preventative services. We aim to have our patient’s current on all applicable health screening services.
It is our goal to communicate with patients clearly and comprehensively in their language of choice so they understand their medical conditions. We make sure to include patient, family, and caregiver education for self-management support to promote active participation in the patient’s care.
We make every effort to schedule our primary care patients with their own personal provider. If this is your first time coming to our Medical Home for primary care, please review our provider descriptions below to choose the staff member that you feel will best fit your personal needs.
PATIENT AND CAREGIVER RESPONSIBILITIES
We ask that patients remain active participants in their health by giving our Medical Home current information including:
- List of all prescription and over the counter medications
- Medical Records from all medical encounters including office visits, diagnostic tests, ER visits, hospitalizations and out-patient treatments
- The best form of communication to contact the patient and caregiver
- Up to date insurance information
- Scheduling and maintaining appointments as requested by the PCP
- Changes in health status
PRIMARY CARE PROVIDERS

Julia Kaci, MD
Dr. Kaci graduated from the University of Tirana School of Medicine, then completed her residency in Family Medicine followed by a fellowship in Geriatrics at the University of Florida in Gainesville, FL. She is board certified in Family Medicine and holds a Certificate of Added Qualifications in Geriatrics. Her areas of interest are nutrition and women’s health. She lives in Westchester with her husband and three kids.

We are here for our patients 24 hours a day, 7 days a week. Our normal office hours are 8:30 am to 4:30 pm Monday thru Friday (with extended hours on Saturday mornings 9-10am). Please call us at 914-493-8148 if you would like to make an appointment or have a question for your provider. During office hours, our staff members will help you get the answers you need. If you call after office hours our medical answering service will route your call to the on-call primary care provider in a timely manner.
