This 6- workshop series aims to provide caregivers with the directions for creating a transition plan for who next will be responsible for supporting their loved one. At the end of the workshop series, the caregiver will have the knowledge to create a caregiver transition plan, which will include the systems, people and processes needed to successfully support their loved one so that they live life to the fullest. The process of transferring knowledge to new caregivers and circle of supporters will be an important aspect of the planning process. Our first workshop focuses on systems. Other workshops will cover people and staffing, financial planning, and housing.
Workshop 1: Overview and Systems
- What should caregivers of people with intellectual/developmental disabilities be thinking about as they plan for the time when they are older and beyond?
- What are the key systems that should be in place (eg SSA, OPWDD, Self-Direction), and what are the associated paperwork and information requirements from these systems that future caregivers need to be informed about? What legal documentatio needs to be in place?
