How to Request a Copy of My Records at WIHD

To obtain a copy of records, the patient will need to fill out a Consent for Disclosure of Protected Health Information (PHI) Form (also available in Spanish) or an equivalent to this document which gives permission for the person requesting to obtain the documents.

If the patient is a minor or is not competent to give consent, the signature of a parent, guardian or other legal representative will be required. Please note that in certain circumstances legal guardianship will require proof.

All requests must be made in writing.

How will my records be released?

There are several methods WIHD can use to release records.

  1. Patient Portal – for most clinical health services (not including Early Intervention) WIHD’s Patient Portal is available for every patient.  The patient portal has the following benefits:  secure, some health information is available, visible past and future visit schedule, and records can be requested.   2. Fax, mail, secure email –Records can be sent via these methods as long as this information is in the Consent for Disclosure of Protected Health Information (PHI) Form (also available in Spanish) signed by the patient or legal guardian/representative.
  3. In Person –Records can be picked up in person by the patient or an authorized individual.  A picture ID will be required before records are released.

How long does it take to get my records?

All requests are worked in the order they are received.  While effort is made to complete requests as soon as possible please allow up to 7-10 business days to process.
