Information for Former LEND Trainees

As you are aware, we are required by our federal funding agency to maintain and update our contact information and database on all trainees who have completed the LEND Program. In order to make this process as user-friendly as possible, we are providing two ways to accomplish this—you can update your information yourself electronically, or we can send you a paper survey that you can complete and return to us. Either way, this will take no more than 10 minutes of your time.

Whichever method you choose to use, you need to contact Cathy Yankou at 914-493-8175 or e-mail her at CYankou@wihd.org. If you elect to enter your updated information directly into the electronic database, she will provide you with a login name and password. Or, if you wish, she will mail you a paper survey you can complete and return to us.
