Early Intervention Service Coordination

Overview

The Family Connection Service Coordination Program is a contract service with the Westchester County Department of Health and provides initial and ongoing service coordination for families of infants and toddlers with disabilities or developmental delays in Westchester County.  These services are provided by a staff of trained professionals who are parents of children with special needs.  Families receive assistance in eligibility and in accessing, coordinating and monitoring services.
